The Community Support Specialist conducts assessments, coordinates independent living skills program plans, and provides assistance, supervision, and training in the area of independent living. The Community Support Specialist will assist clients in locating and gaining access to housing and necessary waiver services as well as medical, social, educational or other needed services regardless of the funding source. The Community Support Specialist acts as an advocate for individuals and their needs. 

The Community Support Specialist must be able to communicate effectively with clients, families, outside agencies and coworkers. They must maintain and foster cordial relationships and optimal communication and displays positive, supportive and cooperative conduct. They must be able to communicate effectively both verbally as well as in writing; listening accurately and responsively. The Community Support Specialist seeks/pursues services for non-English speaking clients as appropriate.

The Community Support Specialist must be able to work independently without direction or supervision immediately available. Maintaining a good driving record and the ability to drive company vehicles may also be required.

Requirements:

  • Bachelor of Arts preferred
  • At least 2 years of experience with person with disabilities and/or in Health and Human Services.
  • Experience with person centered planning approaches.
  • Knowledge of disabilities/chronic illnesses.
  • Knowledge of independent living skills management.
  • Excellent communication skills, written and verbal.
  • Ability to coordinate activities and services.
  • Ability to read, write and speak the English language.
  • Ability to travel among agencies; if driving, holds a valid license and maintains a safe driving record.
  • Ability to work with diverse clients.
  • Ability to work flexible hours.

Benefit package available for staff who work an average of 30 hours or more per week. PTO accrual begins the day you start employment.
